the photo is the video identified as Rick Jarrard is actually a photo of Paul Kantner of the Jefferson Airplane/starhip from the Jefferson Starship period over 10 or 15 years after the airplane worked with Jarard on Surrealistic Pillow.

sorry i don't know what the easy fix is for the video, but i'd fix it before mastering dvds.
